Athletic Forms and Information    ****** NEW PROCESS******

Memorial Middle School is transitioning to an online forum to complete required athletic forms.  The high school has used this process in the past and it has streamlined the process tremendously for both parents and schools. Even if students are on the fence about playing a sport, or aren’t planning to participate in a sport until the Spring, now is a great time to get a head start on the paperwork.  

 

Before beginning to fill out the documents you will need your student’s ID Number (found on Infinite Campus) and insurance information.  This process will take about 10 minutes for you and your child to complete and the link to the online site, RankOne, is found here. Another feature that this online service provides is that parents have the ability to sign up for text alerts when a game or practice is cancelled.  The process to sign up for those alerts can be found by following the “schedules” link below.

The only piece of information that needs to be submitted hard-copy to the school is proof of your child’s physical from their Doctor’s office.  Proof is good for two years and can be mailed to Memorial
